If you've been in hospital and need help to continue living independently at home, we have a dedicated team, based at the Royal Alexandra Hospital, who can support you and make sure you don't need to stay in hospital any longer than necessary.
Hospital discharge assessment and care plan
It's very important that we put in place the right support to help you return home safely and live independently in the community, so we'll carry out an assessment of your needs and any risks.
This will be done in consultation with you, your family and, if necessary, healthcare workers such as your GP, district nurse, physiotherapist, occupational therapist, home carer, dietician, community psychiatric nurse or other relevant specialists.
Based on this assessment, a care manager will discuss their recommendations along with the outcomes you've identified as being important to you and from this, we'll develop a care plan for you.
If your needs are more complex and it's agreed that an alternative to returning home is necessary, we'll work with our health colleagues to offer guidance and assistance to you and your family to support your move on from hospital.
