Care at Home can provide personal care and practical support to help you live independently in your own home.
Reablement
This is an important part of our service, as it is designed to help you remain as independent as possible by supporting you to regain skills and confidence if you have been ill, had an accident, have a disability, or have recently found it more difficult to manage daily living tasks.
Our goal is to help you live as independent and full a life as possible within your home and community. By "doing with you" rather than "doing for you", we will encourage and motivate you to regain confidence, ability, and skills to live safely at home. We will usually work with you for six weeks to understand which tasks are important to you and we will help you set timescales to relearn these by.
We will listen carefully to you to make sure you have the support you need and give you choice and flexibility. We can also support informal carers, recognising their needs and rights, and if you need it, we can provide appropriate equipment and assisting technology to make your life easier. Near the end of your reablement journey, we will review your progress and check if you need longer-term support.
Care at Home
If you are assessed as requiring longer-term support, a referral for Care at Home will be made. Our staff can support you to maintain independence whist assisting you with tasks such as getting out of bed, making breakfast, maintaining personal hygiene and continence care, getting dressed and supporting you with your medication.
You will be fully involved in care and support planning to ensure that the support provided is tailored to your individual needs, preferences, and values, leading to better outcomes. Depending on where you live, you might receive your Care at Home Service from one of our external partner organisations.

Emergency social work services provide assessment and intervention in emergency situations to relieve acute risk and are too urgent to wait for social work offices to re-open.
